Strange as it seems, I have played 2 season of ML and World Cup matches, till date I have not seen a penalty. Am I missing a patch or something? Advice needed.

have you patched the game with official 1.03 konami patch?

Some people told with this patch the penalty issue is solved...other said not... Try by yourself...

PS: delete yours saved files before apply the patch, and restart new ML or new world cup matches...
 
the penalty solved, but still very rare if you played in Top Player mode, as the CPU is extremely precised in the tackling timing.
